New Patient Registration
If you are new to the area (please check our practice boundary map) and wish to register with one of our GP’s please ask at our reception. Please bring with you:
- your passport or some other form of photo ID
- your NHS number if known
- proof of address (a recent utility bill or similar)
Following your registration you will be required to have a new patient check to enable your registration to be fully complete. An appointment will be offered with our healthcare support worker during your registration.
Medical treatment is available from the date of registration. Please contact reception for further information.
You do not need your NHS Number to register as a patient. However, in order for us to achieve a smooth and error free registration your NHS Number is an integral part of your care. If you do not know your NHS Number, information can be found at:
